Tooth Loss Tracks With Blood Markers of Neurodegeneration

Clinical question: does tooth loss (edentulism) predict cognitive decline and dementia, and is this association mediated by blood-based markers of neurodegeneration?
Methodology: 4,238 participants aged 50+ from the nationally representative US Health and Retirement Study (HRS). Edentulism status was recorded in 2012, blood biomarkers — neurofilament light chain (NfL), glial fibrillary acidic protein (GFAP), phosphorylated tau-181, and amyloid-β 42/40 — were measured in 2016, and cognitive outcomes were tracked from 2012 to 2022. Mediation analysis tested how much of the edentulism-cognition link the biomarkers explained.
Findings: edentulism was associated with higher NfL and GFAP, but not with pTau-181 or amyloid-β, and predicted a 49% higher risk of cognitive impairment (HR 1.49) and a 28% higher risk of dementia (HR 1.28) over 10 years. GFAP mediated 9.8% of the cognitive impairment association and 12.0% of the dementia association; NfL mediated 9.8% of the dementia association only.
Clinical relevance: this is an observational, associative finding — it does not establish that tooth loss causes dementia. But it strengthens the case for tooth retention and prosthetic rehabilitation as part of a broader conversation about healthy ageing, and gives periodontists and prosthodontists a concrete data point to share with physicians managing at-risk patients.
Why it matters in practice
In this 10-year US cohort, edentulism predicted a 28-49% higher risk of cognitive impairment and dementia, partly explained by elevated blood biomarkers of neural injury — a reason to flag tooth loss as a signal worth discussing in shared care with physicians.
